The person named above (you) hereby affirm that by submitting this form you are participating in this project and/or production voluntarily.  You understand and assume sole responsibility for your participation and any resulting bodily injury or physical damage that may occur.  This includes but not limited to bodily injury or physical damage resulting from either your acts or omissions; or simply attendant to your presence and participation in this project and/or production.

As applicable, you grant permission to the producer to take and use your contribution(s) to the project/production, inclusive of the use of audio and visual images of your performance and/or participation in this project or productions.  Visual and audio images constitute any type of recording regardless of the medium used, including but not limited to photographs, digital images, drawings, renderings, voices, sounds, video recordings, audio clips or accompanying written descriptions.  You agree and acknowledge that the producer possesses a shop right in the project/production and may further utilize the images and production for equational purposes.

The images may be used in any manner or medium without notifying me, including web sites, publications, promotions, broadcasts, advertisements, posters and theater slides, as well as other uses and/or publications, presentations and performances.  You waive any right to inspect or approve the finished images or any printed or electronic matter that may be used with them, or to be compensated for them.  You understand that you will receive no consideration, monetary or otherwise, regardless of whether or not the project, or any part thereof, is ever published.

You expressly release and hold harmless the producer, his/her employees and agents, from any and all claims, monetary damages, injuries, fines, penalties, awards, judgments, and liability of any kind or nature that may occur or arise as a result of your presence or participation on location, or involvement in the project or production, or in connection with the taking or use of images attendant to the project/production.
